Iliotibial band syndrome
A tendon problem often develops gradually and increases in severity over time. The course of symptoms is often recognisable and goes as follows:
- Complaints at the start of an activity that gradually disappear. This is followed by an after-reaction when the activity is over.
- Complaints at start of activity that gradually disappear but now recur during activity already.
- Complaints that do not go away after the start of an activity and may be present even at rest.

Cause
The iliotibial band is a band made of tendon tissue. The ITB runs from the side of the hip/ thigh to the outside of the knee. In most cases, irritation of the iliotibial band causes pain on the side of the knee. Irritation of this ligament is often caused by overuse. This type of overuse occurs in runners but certainly also in cyclists. Because the iliotibial band is made of tendon tissue, recovery is different from muscle injuries. Without proper guidance, tendon tissue recovery can take a long time.
Complaints and symptoms
As described earlier, irritation of the iliotibial band causes pains on the side of the knee. There is often pain on slight flexion of the knee. This pain is often described as stabbing. In addition, while stretching and bending the knee, a clicking/snapping sensation or sound may be experienced. In some cases, the knee may be somewhat swollen. Sometimes a stiff feeling is present at the side of the thigh/hip.
Diagnosis and examination
To diagnose iliotibial band syndrome, an interview is held during the first treatment. Using a number of questions and tests, the physiotherapist will know whether you have illiotibial band syndrome or another condition. The physiotherapist will explain what is going on and how best to treat this condition.
